Abstract
Antimicrobial stewardship is an essential component of transplant programs to improve patient outcomes, reduce the threat of antimicrobial resistance, and lower the risk of drug toxicity. A multidisciplinary approach that leverages the existing team-based infrastructure of transplant programs and encompasses partnership with pharmacy, infection prevention, and microbiology is important for successful implementation. This chapter outlines the importance of stewardship in transplantation, describes implementation strategies, and provides examples of syndrome-or drug-specific antibiotic, antiviral, and antifungal stewardship targets for transplant patients. Finally, program process and outcome metrics should be chosen to assess the impact of interventions and identify opportunities for improvement. © Springer Nature Switzerland AG 2021.
