Abstract
Follow-up reminder alerts within the electronic medical record (EMR) can be successfully implemented into Family Practice. Adequate diabetes management can be completed through continuous monitoring by the primary care provider and with consistent follow-up. Practice and disease management should not rely solely on the patient to complete follow-up recommendations. The healthcare professional has a role and responsibility to contact patients about follow-up, laboratory and imaging results, and disease management. By establishing a simple, consistent and reliable discharge process and reminder alert system within the EMR, patient safety and disease management can be improved.
