Abstract
Physical examination showed controlled blood pressure with signs of heart failure: basal lung crackles, elevated jugular venous pressure, bilateral leg edema and S4 gallop. Bone marrow biopsy with cytogenetic studies and immunohistochemistry showed no evidence of lymphoma or myeloma while in-situ hybridization showed kappa-restricted plasma cells, establishing the diagnosis of cardiac amyloidosis (AL type).
