Abstract
Rural populations face disproportionately poor access to surgical care due to geographic, workforce, and infrastructure barriers. Technological innovations, including telehealth, remote patient monitoring, and clinical decision support tools, offer potential solutions, but implementation in rural settings remains limited. This article explores how technology can be leveraged to expand surgical access and improve outcomes in rural America.
A targeted review of the literature, policy developments, and emerging digital health models relevant to rural surgical care was conducted. Opportunities and challenges were identified through thematic analysis of multisite studies, pilot initiatives, and federal programs.
Telehealth and remote monitoring can support preoperative and postoperative care while reducing travel burden. Decision support tools embedded in electronic health records may enhance clinical decision-making, though many lack rural-specific adaptability. Key barriers to adoption include inadequate broadband, limited technical capacity, and reimbursement restrictions. Federal initiatives such as the BEAD program and expanded Medicare telehealth coverage may help address these gaps. However, rural customization, stakeholder input, and sustained policy support are essential to scale these innovations effectively.
Technology holds substantial promise to improve surgical access and outcomes in rural communities, but intentional design and policy support are essential. Investments in broadband, cross-state licensure reform, targeted reimbursement, and rural-focused development are critical to ensuring digital health tools translate into real improvements in rural surgical care.
